Eligibility Requirements

  1. You must be at least 18 years old.

  2. Proficiency in English (speaking, reading, writing) is required. 

  3. Minimum education: high school + at least two years of college (or equivalent) is needed. 

  4. Graduate from a medical school approved by the Vermont Board (LCME- or CACMS-accredited) or a similarly acceptable school. 

  5. Complete postgraduate training (residency):

    • For U.S./Canadian medical school grads: typically two years of ACGME- or equivalent-accredited training. 

  6. Pass a licensing examination accepted by Vermont (such as USMLE). 

  7. For international graduates: ECFMG certification is required (unless a “fifth-pathway” medical school). 

  8. Demonstrate good moral character and professional competence. 

  9. Submit a National Practitioner Data Bank (NPDB) self-query report

  10. Provide primary-source verification of education and training:

    • Medical school transcripts. 

    • Residency training verification. 

  11. Undergo criminal background check / identity verification as required by the Board.

Application Process

  1. Apply through the Vermont Department of Health – Board of Medical Practice online portal.

  2. Download and submit all required forms (check the “Applications, Licensing & Fees” page):

    • Initial license application forms 

    • Training license / limited license forms (if applying during residency) 

    • Reinstatement application (if needed) 

  3. Pay the application fee: for an initial MD license it’s US$ 650.

  4. Submit supporting documents: identity proof, education, training, exam scores, NPDB, etc. 

  5. Use the Federation Credentials Verification Service (FCVS) if available — this can simplify verification. 

  6. Monitor your application status via Vermont’s e License system

Interstate (Compact) Licensing

  1. Vermont used to participate in the Interstate Medical Licensure Compact (IMLC) per older documents.

  2. However, check current Board status — compact laws may have changed, so verify on the Vermont licensing portal before applying via compact.

License Renewal

  1. Full physician licenses expire on November 30 of even-numbered years

  2. Renewal requires attesting to CME completion and paying the renewal fee.

Continuing Education

  1. Physicians must complete 30 hours of CME every 2 years

  2. Required topics include: hospice, palliative care, pain management: at least 1 CME hour per cycle.

  3. For physicians with a DEA registration (i.e., can prescribe controlled substances): at least 2 CME hours on prescribing controlled substances are required. 

  4. CME documentation need not be submitted at every renewal, but you must retain proof for up to 4 years, as audits may happen. 

  5. For first-time licensees licensed for less than a full 2-year cycle, there may be reduced requirements (e.g., 15 CME hours if licensed for ≥ 1 year).

Additional / Special Licenses

  1. Training License: For interns, residents, or fellows — lower fee, limited-duration license. 

  2. Faculty License: Vermont may grant a license to full-time faculty at a Vermont medical school under certain conditions.

Processing Time

  1. Applications are reviewed only once all required documentation is received.

  2. Incomplete applications will not be processed until missing items are submitted via eLicense. 

  3. Watch for Board meeting schedules, especially for licensure committee approval if non-standard documentation is involved.
