Apply online through the Florida Department of Health / Board of Medicine portal.
Pay the required application fee, licensing fee, and other applicable fees.
Upload or submit all required documentation:
Medical school transcripts
Residency verification
License verification (if applying by endorsement)
Identity proof
NPDB report
Complete fingerprinting via Livescan for state and national background checks.
If there are any disciplinary, criminal, or malpractice disclosures on your application, provide explanatory documentation.
Once all requirements are met and approved, a license will be issued.
Florida does not require a collaborating physician for prescribing authority.
To prescribe controlled substances:
Apply for a Florida Controlled Substance Registration (CSR).
Register with Florida’s Prescription Drug Monitoring Program (PDMP).
Maintain a DEA registration.
Physicians must complete 38 hours of CME every 2 years.
Additional required CME:
2 hours in medical error (must cover the 5 most misdiagnosed conditions).
If you are registered with the DEA and prescribe controlled substances: 2 hours in controlled substance prescribing.
For your first biennial renewal, there is an exemption: only 2 hours medical error + 1 hour HIV/AIDS are required.
The application process can take several months depending on credential verification, background check, and application completeness.
After approval, licenses may be mailed within about 10 business days
