All postgraduate training (residency/fellowship)
Submit verification of all other medical licenses held in other jurisdictions.
Complete a fingerprint-based criminal background check.
Pass a written licensing examination acceptable to the Idaho Board (for initial license).
Fill out the physician application form provided by the Idaho Board of Medicine / DOPL.
Disclose any malpractice settlements, disciplinary actions, or other relevant history.
Provide a signed affidavit and complete the background check portion.
Submit primary-source documents for:
Education (medical school)
Residency/fellowship training
Licenses from other states
Request NPDB (National Practitioner Data Bank) report or self-query and submit it if required.
Pay the application / licensing fees. (As specified on the DOPL / Board of Medicine site.)
