Hold a valid, unencumbered U.S. medical license, or apply for a new Idaho medical license (MD or DO).
Graduate from an accredited medical school (MD or DO).
Complete a postgraduate training program (residency) that the Idaho Board of Medicine deems acceptable.
For international medical graduates (outside U.S./Canada):
Submit ECFMG certification or proof of passing Board-acceptable exams.
Provide original documentation from your medical school.
Meet additional Board-specified criteria (such as approved postgraduate training or board certification) if your school is not WFME/FAIMER-listed.
Provide primary-source verification of:
- Medical school education
- All postgraduate training (residency/fellowship)
- Submit verification of all other medical licenses held in other jurisdictions.

- - Complete a fingerprint-based criminal background check.
  - Pass a written licensing examination acceptable to the Idaho Board (for initial license).

Fill out the physician application form provided by the Idaho Board of Medicine / DOPL.
Disclose any malpractice settlements, disciplinary actions, or other relevant history.
Provide a signed affidavit and complete the background check portion.
Submit primary-source documents for:
- Education (medical school)
- Residency/fellowship training
- Licenses from other states
Request NPDB (National Practitioner Data Bank) report or self-query and submit it if required.
Pay the application / licensing fees. (As specified on the DOPL / Board of Medicine site.)

To prescribe controlled substances in Idaho, you must apply for the Idaho Controlled Substance Registration (CSR).
Once you have the CSR, you must obtain a DEA registration within 45 days.
CSR is required per practice address: each Idaho practice location where you prescribe/dispense controlled substances needs its own CSR.

Physicians licensed in Idaho must complete at least 40 hours of practice-relevant, Category 1 CME every 2 years.
At license renewal, you must attest to having completed the required CME.
The Board may accept board certification or recertification (ABMS / AOA / Canadian equivalent) in lieu of CME for that renewal cycle.
Exemptions may be granted for full-time residents or fellows at accredited training programs.
If you do not meet CME requirements, the Board may limit, condition, suspend, or refuse to renew your license.

Typical licensing timeline: 60–120 days from application to issuance, depending on credential verification and background check.
All education, training, work history, and previous licenses (past 5 years) must be verified.
