Hold a Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from a recognized/approved medical school.
For international medical graduates: either:
Submit ECFMG certification, or
Provide evidence of postgraduate training in an acceptable U.S./Canadian institution.
Pass a licensing examination (USMLE / COMLEX or equivalent) as required by the Indiana Medical Licensing Board.
Be physically and mentally capable of practicing medicine competently.
Undergo a criminal background check (fingerprinting) during the application process.
Provide verification of all other medical (or related) licenses held in other jurisdictions.
Submit a National Practitioner Data Bank (NPDB) self-query report (or equivalent as required).
Apply via Indiana Professional Licensing Agency (PLA) through the MyLicense portal.
Fill out the application, including full education, training, and license-history details.
Pay the application fee (e.g., $250 for MD/DO) at the time of application.
If applicable, apply for a temporary permit:
Non-resident or out-of-state licensed physicians may get a 90-day temporary permit.
Submit all required supporting documents:
Medical school transcripts / diploma
Residency / postgraduate training verification
Evidence for any “yes” responses (disciplinary, malpractice) on application.
If name has changed, provide legal documentation of name change.
For foreign graduates: submit ECFMG certificate as needed.
To prescribe controlled substances in Indiana, you need a Controlled Substance Registration (CSR) issued by PLA.
CSR application fee: US$ 60 for initial application.
CSR is tied to a specific Indiana practice address, so P.O. boxes are not accepted.
To renew CSR, a physician’s primary license must be active.
A DEA registration is also required to prescribe controlled substances.
As of July 1, 2025, completing 2 CME hours on opioid prescribing is not required for initial CSR or renewal.
Physician licenses in Indiana expire on October 31st of odd-numbered years.
Renewal fee for MD/DO license: US$ 200 for active, US$ 100 for inactive status.
Renew through MyLicense.IN.gov.
CSR renewal must be done separately from the medical license renewal.
