Eligibility Requirements

  1. Hold a Doctor of Medicine (MD) or Doctor of Osteopathic Medicine (DO) degree from a recognized/approved medical school. 

  2. For international medical graduates: either:

    • Submit ECFMG certification, or

    • Provide evidence of postgraduate training in an acceptable U.S./Canadian institution. 

  3. Pass a licensing examination (USMLE / COMLEX or equivalent) as required by the Indiana Medical Licensing Board. 

  4. Be physically and mentally capable of practicing medicine competently. 

  5. Undergo a criminal background check (fingerprinting) during the application process. 

  6. Provide verification of all other medical (or related) licenses held in other jurisdictions. 

  7. Submit a National Practitioner Data Bank (NPDB) self-query report (or equivalent as required).

Application Process

  1. Apply via Indiana Professional Licensing Agency (PLA) through the MyLicense portal. 

  2. Fill out the application, including full education, training, and license-history details. 

  3. Pay the application fee (e.g., $250 for MD/DO) at the time of application. 

  4. If applicable, apply for a temporary permit:

    • Non-resident or out-of-state licensed physicians may get a 90-day temporary permit. 

  5. Submit all required supporting documents:

    • Medical school transcripts / diploma 

    • Residency / postgraduate training verification 

    • Evidence for any “yes” responses (disciplinary, malpractice) on application. 

  6. If name has changed, provide legal documentation of name change. 

  7. For foreign graduates: submit ECFMG certificate as needed.

Controlled Substance Registration & Prescribing

  1. To prescribe controlled substances in Indiana, you need a Controlled Substance Registration (CSR) issued by PLA. 

  2. CSR application fee: US$ 60 for initial application. 

  3. CSR is tied to a specific Indiana practice address, so P.O. boxes are not accepted. 

  4. To renew CSR, a physician’s primary license must be active

  5. A DEA registration is also required to prescribe controlled substances. 

  6. As of July 1, 2025, completing 2 CME hours on opioid prescribing is not required for initial CSR or renewal.

Continuing Education Requirements

  1. Indiana does not require general CME for physician license renewal. 
  2. But, if you have a CSR, you must complete 2 credit hours relating to opioid prescribing and opioid abuse during your renewal cycle.

License Renewal

  1. Physician licenses in Indiana expire on October 31st of odd-numbered years

  2. Renewal fee for MD/DO license: US$ 200 for active, US$ 100 for inactive status. 

  3. Renew through MyLicense.IN.gov

  4. CSR renewal must be done separately from the medical license renewal.

Interstate Licensing / Compact

  1. Indiana participates in the Interstate Medical Licensure Compact (IMLC)
  2. You can apply via IMLC if you’re eligible for compact licensure.

Processing Time

  1. The board accepts the Uniform Application (UA) for physician licensure, which simplifies multi-state applications. 
  2. The Board might require you to appear in person for certain waiver requests (e.g., if medical school or training is not standard). 
  3. All “yes” answers to disciplinary or malpractice questions must be fully explained with supporting documentation.
